CD4 Cells

CD4 Cells
Treatment centers need consulting rooms, offices, waiting rooms and secure storage for valuable drugs. They also require basic facilities to test for anemia, pregnancy, renal function, white blood cell counts and other factors that may affect treatment.
In developed countries, decisions about when to start treatment are based on the results of clinical tests called the CD4 test and the viral load test. Ideally these tests should be used everywhere, but in many parts of the world they are currently unavailable, as they require expensive equipment, electricity and trained technicians. In theory, decisions about when to start treatment may be based on symptoms alone. However in practice some treatment programmers provide medication only to people who have had a CD4 count.
There are two primary types of T-cells.
1. CD4 Cells-These cells have molecules called CD4 on its surface. These "helper" cells initiate the body's response to invading micro-organisms such as viruses.
2.CD8 Cells-These types of T-cells have a molecule called CD8 on their surface. CD8 cells destroy cells that have been infected with foreign invading micro-organisms. CD8 cells also produce antiviral substances (antibodies) that help fight off the foreign invader.
3.Normal values-In a healthy adult, a normal CD4 count can vary a great deal but is typically 600 to 1200 cells per cubic millimeter of blood
Between 600 and 350- In an HIV+ person, this range is considered "very good".
HIV medications are typically not be indicated.
Between 350 and 200-The immune system is weakened and therefore the HIV+ person may be at increased risk for infection and illness.
Function of T helper cells: Antigen presenting cells present antigen on their Class II MHC molecules. Helper T cells recognize these, with the help of their expression of CD4 co-receptor. The activation of a resting helper T cell causes it to release cytokines and other stimulatory signals. The importance of CD4 T cells is demonstrated with infection.
It has been proposed that during the non-symptomatic phase of infection, the virus has a relatively low affinity towards T cells (and has a higher affinity for macrophages), resulting in a slow kill rate of CD4+ T cells by the immune system. This is initially compensated for via the production of new helper T cells from thymus. Once the virus becomes T-tropic, it begins to infect CD4+ T cells far more efficiently, and

the immune system is overwhelmed.
At this point, functional CD4+ T cell levels begin to decrease, eventually to a point where the CD4+ T cell population is too small to recognize the full range antigens that could potentially be detected. The lack of full antigen cover results in the core symptoms of acquired immune deficiency syndrome (AIDS).CD4 T cell depletion during AIDS allows various pathogens to escape T cell recognition, thus allowing opportunistic infections that would normally elicit a helper T cell response to bypass the immune system. While these complete bypass situations only occur when the helper T cell response is absolutely necessary for infection clearance, most infections increase in severity and/or duration because the immune system's helper T cells provide a weaker contribution to a less efficient immune response.
Two components of the immune system are particularly affected in AIDS, due to its CD4+ T cell dependency:
CD8+ T cells are not stimulated as effectively during the AIDS period of infection, making AIDS patients very susceptible to most viruses, including itself. This decline in killing of CD4+ T cells results in the virus being produced for a longer period (the infected CD4+ T cells are not killed as quickly), increasing the proliferation of the virus, and accelerating the development of the disease.
Antibody class switching declines significantly once helper T cell function fails. The immune system loses its ability to improve the affinity of their antibodies, and are unable to generate B cells that can produce antibody groups such as IgG and IgA. These effects are primarily due to the loss of any helper T cell that can interact with the B lymphocyte correctly. Another symptom of AIDS is the reduction in antibody levels due to a decrease in Th2 cytokines (and less interactions by helper T cells). All of these complications result in an increased susceptibility to aggressive bacterial infections, especially in areas of the body not accessible by IgM antibodies.
If the patient does not respond to Treatment they will succumb usually to either cancers or infections; the immune system finally reaches a point where it is no longer coordinated or stimulated enough to deal with the disease.

Wake-up stroke victims can receive clot-busting drugs, study says
Stroke victims who are treated with blood-clot dissolving medications often recover better than people who don't receive the drugs. The medications, called tissue plasminogen activators, must be given within 4.5 hours of the first symptoms of stroke.
